Shirley A. Sample, age 99 of Montevallo, passed away Wednesday, Dec. 26, 2012, in Alabaster.

She is survived by her daughter, Betty S. Cox; sons, J. Sidney Sample, Lloyd G. Sample and David L. Sample; sister, Nelda Martin; brother, Bob Anderson; 12 grandchildren; 22 great-grandchildren; and several great-great-grandchildren.

Visitation was held Friday, Dec. 28, 2012 from 6–8 p.m. at Providence Baptist Church No. 1.

Funeral services will be held Saturday, Dec. 29, at 11 a.m. at the church with the Rev. Allen Foster, Dr. Sid Sample and Dr. Sid Nichols officiating.

She will lie in state one hour prior to the service.

Burial will follow in the adjoining cemetery.

In lieu of flowers, donations may be made to: Providence Baptist Church No. 1 Building Fund, 190 County Road 46, Montevallo, AL 35115.
