Lakeshore Foundation in Birmingham and the National Center on Health, Physical Activity and Disability are asking for information about how COVID-19 is impacting  community members with disabilities and chronic conditions.

A survey has been launched and will be available through April 17.

According to a press release from Lakeshore Foundation, the organization is using the survey “to identify what people with disabilities and chronic conditions” need most during the COVID-19 crisis.

“Lakeshore is working locally in Alabama and Jefferson County to gather this information and disseminate it to the Unified Incident Command Center for the Jefferson County Health Department where we are the lead on the disability task force and for the state-wide volunteer response efforts,” the organization said. “The information gathered will also be shared with disability stakeholders across the state and nationally to assist with their respond needs and the disability community.”

“People in our community can answer the questions which will help us figure out the best ways to support the community during this time,” the release stated. “The questions help us understand what is most needed, but if anyone feels uncomfortable answering a question, they can skip it. All answers are anonymous.”
